DREAM Enrichment Case Manager

The DREAM Enrichment Case Manger supports individuals with disabilities in building independence and improving their quality of life. This role centers on person-centered planning, daily living support, and coordination of services to help each individual reach their personal goals.

Case Managers work closely with individuals served, staff, and team members to create and maintain individualized plans, ensure services are delivered effectively, and provide guidance to support staff. The position combines hands-on support, oversight, and documentation, with a strong emphasis on dignity, inclusion, and individualized care.

Essential Functions

1. Facilitate and implement personal planning processes

2. Write Individual Programs

3. Supervise Staff

4. Complete all 245D paperwork

$23/hour
