This is a foundational program for our Caregiver Support Services by reaching out to citizens to have conversations about what matters most to them on how their health care wishes are made and upheld.
The goal of this annual event is to promote, educate and empower our community members on the importance of advance care planning! This is done by talking with loved ones and your own health care providers and even friends—all important steps to making your wishes known in advance of a medical crisis. We anticipate it will be a great community learning project to address “the elephant in the room.”
